Report an Incident

The BGA Safety Management system manual describes how clubs can report and review incidents that are not required to be reported under the established BGA incident & accident reporting system.

Below is a link to the Incident “Occurrence” Report form, or an electronic form which can be filled in online.

Our club has an internal incident reporting system to help identify risks at the club, and establish a process through which significant risks are recognised and addressed. Your report should describe what happened. The next step is to decide what, if anything needs to be done about it.

The club will consider and identify reported incidents which are significant. Potential impact and frequency of occurrences may require positive mitigating action within the club.

Here are some examples of incidents best handled by the club:

– Inadequate equipment or glider Daily Inspections
– Potential collision taking off or landing
– Hazardous circuit or approach
– Poor handling
– Groundloop without damage
– Poor parking/ ground collision risk
– Hangar rash
– Pedestrians on the airfield
– Out of date or incomplete paperwork
– Airmanship issues

Please submit the report below. It will be sent to the CFI and Safety Officer for consideration and will be recorded in the Club database. You can also download the occurrence form and post in the box in the Clubhouse if you prefer.
